Your maintenance lead wheels a plasma cutter up to a 55-gallon drum that used to hold paint. The drum looks empty. It isn’t. One arc into the lid and the vapor inside lights off — and now your shop has a burn victim, a fire crew en route, and a worksite that will be shut down for weeks.

That is not a hypothetical. On July 15, a 24-year-old welder at a property in Indian Hill, in Hamilton County, Ohio, was cutting the top off a drum that had previously held paint when an explosion was reported, according to FOX19 Cincinnati. He died from his injuries the next day. Radio traffic indicated he was using a plasma machine on the drum when it happened.

What happened in Indian Hill

The worker was doing routine fabrication work. Cutting the lid off a used drum is the kind of task that happens in maintenance shops every day. The hazard was invisible. A drum that held a flammable or combustible liquid keeps vapor in its headspace long after the liquid is gone. Heat from cutting gives that vapor the one thing it was missing.

The result was a reported explosion and severe burns. The Hamilton County Coroner’s Office confirmed the worker died at a Cincinnati hospital the following day. He was 24 years old.

This is the pattern behind a large share of hot-work fires and explosions. Not exotic industrial processes. Ordinary tasks — cutting, grinding, welding — done on or near containers, walls, and equipment that carry a hidden fuel load. The work takes ten minutes. The consequences do not.

What the rules actually require

OSHA regulates welding, cutting, and brazing under specific standards for general industry and construction, including 29 CFR 1910.252. Before any hot work begins near a container that held flammable material, the container has to be cleaned, purged, or otherwise made safe. “It looked empty” is not a method. OSHA’s welding, cutting, and brazing topic page lays out the standards and the known hazards.

NFPA 51B, the Standard for Fire Prevention During Welding, Cutting, and Other Hot Work, adds the fire-side framework most fire codes reference. Under NFPA 51B, hot work near combustibles requires a permit system, a cleared zone — the familiar 35-foot rule — and a designated fire watch. Your local fire code, adopted through the International Fire Code in most jurisdictions, points back to the same duties. Ohio adopts its own fire code, and the authority having jurisdiction — your local fire marshal — interprets how it applies to your site. Adoption and enforcement details vary, so confirm with your AHJ.

When a fire watch gets triggered

A hot work fire watch is not optional paperwork. It is required any time hot work happens where combustibles cannot be fully removed, where wall or floor openings could carry sparks to another area, or where the permit authorizer decides conditions demand it. A sprinkler or alarm impairment during the work raises the stakes further.

After a fire or explosion like the one in Indian Hill, the fire watch question comes back around fast. If the incident damages your suppression or alarm system, or the fire marshal orders restrictions while the site is stabilized, you may be under an impairment watch before the week is out. What a hot work fire watch guard actually does

The guard’s job starts before the first spark. They walk the area with the operator. They check that combustibles within 35 feet are removed or covered with listed blankets or shields. They verify the extinguisher is charged, in reach, and right for the fuel. They confirm containers in the work zone have been properly cleaned and tested — or moved out.

During the work, the guard watches. That is the whole job, and it is a full job. They track where sparks and slag travel, including through floor penetrations and wall openings to spaces the welder cannot see. They hold a means to sound the alarm and call the fire department. They do not sweep floors, fetch tools, or watch two jobs at once.

After the last arc, the guard stays. NFPA 51B calls for a fire watch to remain at least 30 minutes after hot work ends, longer where conditions warrant — and many AHJs extend that window. Smoldering fires in wall cavities and drum residues routinely outlive the work crew’s patience. The guard logs the watch: times, area checks, and any findings. That log is what your insurer and the fire marshal will ask for first.

What the property manager should do next

Start with your permit program. If your site does not issue written hot work permits — with a named permit authorizer, a pre-work inspection, and a signed fire watch assignment — that gap is your first fix. Walk your shop and your contractor list. Ask who cuts, grinds, or welds on site, and where. Pay special attention to any container, tank, or drum in the work area.

Next, set the rule for used containers: no cutting or welding on any drum or tank until it has been cleaned and tested by a qualified method, or the work goes to a contractor equipped for it. Then decide who stands the watch. Your own staff can do it if they are trained, equipped, and genuinely freed from other duties.
